The attached survey was created as a Google Form A few surveys were created with Google Forms and used to collect user expected behavior and feedback.
One survey was for all 5C library workers. It was emailed out to the FCLIBTS and FCLIB listservs, along with being distributed via Slack and also emailed internally at several institutions. We received 15 responses from individuals. Hampshire College library staff did the exercise together and submitted a separate feedback document. The survey is below. Responses
Another survey was created by Mount Holyoke and only completed by Mount Holyoke library staff.
